Abstract :【目的】認知症高齢者の家族介護経験を持つ看護職者の家族介護経験と看護実践との関係を検討するととである。【方法】研究協力者は、家族介護をする以前に、認知症高齢者を看護実践の経験を持ち、離職せずに認知症高齢者を1年以上の家族介護し、現在も継続している40代~50代の看護職者6名であった。研究方法は、半構造化した面接質問紙調査で、過去の記憶が混乱しないよう複数回面接を実施した。分析内容は、家族介護経験前後の看護職者のケアの変化、看護実践の家族介護経験へのいかし方、家族介護経験の看護実践へのいかし方であり、家族介護経験後の看護実践について質的帰納的に分析した。【結果】認知症高齢者の家族介護経験前後の看護聴者のケアは、ケアの対象である対象者本人、家族、同僚で変化していた。看護実践の家族介護経験へのいかし方は、【多様な症状への対処】 【高齢者の生き様の受容】【家族介護の強化】【専門職の役割の活用】があった。家族介護経験の看護実践へのいかし方は、【認知症高齢者の体験世界の受け止め】、【個別的なアセスメントによるケア】、【家族へのケアと介護者との共感】、【介護カのケアマネジメントとサービス利用の促進】、【職場の環境とケア方法の改善】での【地域での看護実践】があった。【結論】家族介護経験と看護実践との関係は、看護実践が家族介護経験にいかされ、家族介護経験は看護実践にいかされていた。

Comment :Objective : This study aimed to investigate the relationship between the experience of being a family care giver and nursing practice in professional settings in nurses caring for elderly individuals with dementia. Methods : Five nurses in their forties and fifties who cared for an elderly family member with dementia for > 1 year without leaving work participated in this study. All 5 nurses had experience with dementia patients in professional settings before providing care to their family member. A technique combining a questionnaire and semi-structured interviews was employed. Each participant was interviewed several times so that consistency of recollection was achieved. Analyzed items were changes in care for dementia patients after gaining experience as a family caregiver; types of nursing practice experience beneficial in caregiving for an elderly family member; types of experience as a family caregiver beneficial in nursing practice in professional settings. Nursing practice after being a family caregiver was qualitatively analyzed using an inductive approach. Resuts : Participants, family members, and colleagues were associated with changes in care for dementia patients after gaining experience as a family care giver. Family caregiving benefited from the following types of experience gained through nursing practice: dealing with various symptoms individually; accepting the way of life of the elderly; reinforcing family caregiving; and utilizing professional roles. Nursing practice in professional settings benefited from the following types of experience as a family caregiver: awareness of dementia patients' experiences; care based on a personalized assessment; empathic feelings toward provision of care for family members and toward caregivers; proactive approaches to management of caregiving capacity and use of services; improvement of the work environment and methods for care; and regional nursing practice. Conclusion : Experience as a family caregiver and nursing practice in professional settings influenced and benefited each other reciprocally.
